  • 3 most recent pay stubs (for those paid monthly)
• Income Ledgers or Profit and Loss statements for self employed
• Child Support Income Verification form from the Office of the Attorney General
• Most Recent Social Security Letter
• Most Recent benefit letter for paid out retirements/pensions
• One month statement for all Checking and Saving Accounts
